Emergency Contact & Family Information: contact details for emergency contacts, family members, or loved ones you authorize us to communicate with, including through Bridge Sessions™ or Circle of Healing™ programming.
1.2 How We Use Your Information
We use your information to respond to inquiries and schedule confidential calls or tours; assess eligibility and determine an appropriate program (e.g., Detox & Stabilization, Signature Programs, Hybrid Programming); provide, coordinate, and document clinical care, including medication management; communicate with your emergency contacts or support system with your consent or as permitted by law; verify insurance and process payment; improve our website and services; and meet legal, regulatory, and licensing obligations. We do not use personal health information for marketing without your explicit, separate consent.
1.3 Disclosure of Information
We do not sell, rent, or trade your personal information. We may disclose information to regulated health professionals directly involved in your care; to emergency contacts or family members only with your consent, except where necessary to prevent serious harm; to insurance providers to verify or process coverage; to regulatory colleges, licensing bodies, or public health authorities where required by law; to third-party service providers bound by confidentiality obligations; or where required by court order or other legal process.
1.4 Indigenous-Informed Care & Cultural Information
Cultural, spiritual, or community information shared as part of Indigenous-informed programming is treated with the same confidentiality protections as all other personal health information and is used solely to support culturally responsive care, with your consent.
