Ongoing support and community connection to help alumni sustain lasting recovery

Breakthrough Recovery Centre provides ongoing aftercare in Canada through the Horizon™ Aftercare Program, offering continued support for graduates navigating life after residential treatment.

Recovery continues long after residential treatment ends. At Breakthrough, this next stage is supported through a weekly psychoeducational group, not left unaddressed once formal programming concludes. This includes ongoing education, accountability, and peer connection, delivered in a structured virtual format that respects each graduate’s independence while keeping meaningful support close at hand.

What Is Horizon™ Aftercare?

Horizon™ Aftercare is a weekly psychoeducational group facilitated by an Addiction and Mental Health Counsellor, designed to help graduates strengthen their recovery and confidently navigate life beyond treatment.

The program includes:

💻 Virtual format delivered through Zoom
🎓 Facilitated by an Addiction and Mental Health Counsellor
⏱️ One-hour sessions

Zoom meeting information and weekly invites are provided prior to discharge, so graduates can transition into aftercare without interruption.

Purpose of Aftercare

  • Assist with challenges during community reintegration
  • Provide ongoing education about addiction and mental health

  • Emotional regulation and stress management
  • Healthy relationships, communication, and boundaries
  • Self-care, wellness, and building daily routines
    Mindfulness, grounding techniques, grief, loss, and resilience

Expectations for Participants

  • Attend sessions on time and participate respectfully
  • Support fellow group members throughout the session
  • Join the meeting from a private location
  • Minimize distractions during the session
  • Participation is encouraged, but individuals may choose their level of involvement and are never required to share personal information they are not comfortable discussing

Confidentiality

  • All participants are expected to respect the privacy of fellow group members
  • Information shared during the group must remain confidential and not be discussed outside the session
  • Exceptions apply only where disclosure is required by law
