At Breakthrough, this next stage is supported through practical guidance, resources, and ongoing planning, not left to chance once residential treatment concludes. This includes housing navigation, employment support, and continued clinical connection, delivered through an individualized plan that helps each client build a strong and resilient life beyond treatment.
